The bolts were missing

The NTSB’s preliminary report finds evidence that the four bolts that hold the door plug in place were not reinstalled following rework on the 737-9 before it was delivered to Alaska Airlines. The preliminary report contains a large collection of detailed images from the aircraft and is one of the most comprehensive preliminary reports we've ever seen.

An Avion Express A320 landing in Vilnius earlier this week veered off the runway during landing (photos), traveling a long distance through the snow-covered grass before taxiing back onto the runway and to the terminal area. The aircraft sustained substantial damage and remains in Vilnius.

Tourists to Antarctica this week got a good view of the coast, but that’s about all they got when their A340 was unable to land due to heavy fog moving over the Wolf’s Fang airfield as they approached. The flight became an 11 hour, 22 minute round trip to Cape Town.
