Det var en såkalt single pilot operasjon i ekstremt dårlig vær:

• Bad flying weather: There was extremely low visibility at the time of the crash, with foggy, cloudy conditions. LA Police grounded its helicopters Sunday morning, and a police spokesperson said the weather conditions did not meet the minimum standards for flying.
• "We extend our sincerest condolences to all those affected by today's Sikorsky S-76B accident in Calabasas, California," the tweet read. "We have been in contact with the NTSB and stand ready to provide assistance and support to the investigative authorities and our customer. Safety is our top priority; if there are any actionable findings from the investigation, we will inform our S-76 customers."
  The aircraft was built in 1991, according to the FAA and registered to Island Express Holding Corp,  a private helicopter transport company.

Om dette var et privat helikopter eller et som drev kommersiell virksomhet, vet jeg ikke nå. Uansett, det er forferdelig trist å se havarier forårsaket av været, ulykker som kanskje kunne vært unngått med to flygere som tar en avgjørelse om flyging eller kansellering/avbrutt flyging, i fellesskap.

To flygere er en ekstra kostnad samt at en kan ta en pax mer dersom det er et ledig sete foran. Det er pengene som rår.
